In-Depth Clinical Development
For years, parents were handed a single DSM-5 support level (Level 1, 2, or 3) that failed to explain why one child spoke at 12 months yet suffered severe sensory and emotional dysregulation, while another child had significant early speech delay yet showed calm emotional regulation and affectionate social motivation.
In July 2025, Litman et al. (Nature Genetics) analyzed genomic and clinical data from more than 5,000 participants in the SPARK autism cohort, identifying four distinct phenotypic classes that we systematically map during our 4-Phase Diagnostic Protocol at PsicoKids & Teens®:
- Social/Behavioral Challenges with Preserved Early Milestones (~37%): Children, teens, and adults who walked and spoke on time (often missed in toddlerhood) but experience significant social-pragmatic challenges, cognitive rigidity, and high rates of co-occurring ADHD or anxiety.
- Mixed ASD with Developmental Delay (~19%): Children with early delays in motor or verbal milestones but low psychiatric dysregulation—who respond dramatically to early high-intensity intervention in our Intensive TRAZOS Program (English ABA & ESDM).
- Moderate Challenges with Regulatory Vulnerability (~34%): Profiles marked by sensory hypersensitivity and emotional dysregulation during transitions.
- Broadly Affected High-Complexity Phenotype (~10%): Associated with high-impact de novo genetic variants requiring close coordination with our Allied Pediatric Neurologists, medical genetics, and Augmentative and Alternative Communication (AAC).
